Forms and Documents Required by But God Ministries

Team Leaders:

• Team Initial Registration Form (Team Leader submits when trip is scheduled and mails $500 deposit to secure the week.) Use this Team Leader Checklist as a guide.
• Team Planning Update Form (Team Leader submits at least 30 days before the trip.)

ALL Participants:

• Participant Information and Release Form (ALL participants submit to register for the trip.) This form will ask you to attach pictures of these documents:
  • 1. Your Passport 
    2. For medical professionals:
       • • Copy of diploma from school (medical, dental, nursing or pharmacy school)
         • Copy of annual license

• Minor’s Travel Permission Form (All Participants Under 18, not traveling with both parents – Download and print this form. Take 2 original signed and notorized copies with you on the trip.)
• Haiti Orientation Document – All Participants read the information and follow all of the guidelines.
• Haiti Packing List/Travel Information – All Participants follow this list and know this arrival information.
